Cyrtanthus labiatus is a widespread, but very rare and localized species, known from three subpopulations. It has an extent of occurrence (EOO) of 5222 km², with no severe threats and is therefore not suspected to be in danger of extinction. It is therefore listed under the IUCN 3.1 Criteria, globally, as Least Concern but is nationally categorised as Rare.

This species is endemic to South Africa, and occurs in the Klein Swartberg, Groot Swartberg and Baviaanskloof mountains.

It is localized to shaded ledges on vertical cliff faces.
There are no known severe threats to Cyrtanthus labiatus.

This is an extremely rare and localized habitat specialist, known from only three, highly disjunct subpopulations. It is possibly overlooked, as its habitat is inaccessible.
