Justification
This species has a limited distribution range and has an extent of occurrence (EOO) of 814 km² and an area of occupancy (AOO) of 64 km². It is known from nine locations which are declining due to illegal collecting of plants from the wild for the specialist succulent trade as well has habitat loss to agriculture and urban expansion. It therefore qualifies as Vulnerable under criterion B.

This species is endemic to the Western Cape Province of South Africa, where it extends from Calitzdorp to Oudtshoorn.

Description
Plants are found on rocky sites in karroid velds.
Threats
A localized and very attractive miniature succulent, this species is popular in horticulture, and field observations indicate that plants are often removed from the wild, and that it has already declined extensively (J.H. Vlok and A.L. Schutte-Vlok pers. obs.). Twenty nine plants of Haworthia truncata var. truncata were included in confiscations between March 2019 and December 2021, many more are likely to have been removed from the wild but not intercepted by law enforcement efforts. In addition, it is threatened by habitat destruction as a result of expanding ostrich farming as well as urban expansion around Calitzdorp and Oudtshoorn.
